To validate registration a valid signature from a parent or guardian is required for all participants.

WITH MY SIGNATURE BELOW, I AM AFFIRMING THE FOLLOWING STATEMENTS TO BE TRUE:

• I understand that teachings in this symposium are based on biblical principles aligned with the Christian faith.
• I understand that NarroWay will not shy away from difficult questions or uncomfortable conversations in regards to sexuality and relationships in this symposium.
• I understand that teens will be allowed to ask questions and receive honest, biblically based answers concerning sensitive topics such as sexuality and relationships.
• I attest that my teen(s) has/have the maturity to participate in this symposium.
• I understand that no late arrivals are permitted. Doors to the NarroWay Theatre will be locked at 2pm with no further admissions allowed.
• I release and indemnify, defend, and hold harmless NarroWay Productions and its officers, members, and volunteers from all liabilities, losses, costs, damages, claims or causes of action of any kind or nature, whatsoever, sustained by my child as a result of or in connection with participation in the event.

Promotional Release

I hereby grant to NarroWay Productions, Inc. the irrevocable right to photograph, videotape and/or record audio while participating in symposium activities and to use these images and/or recordings for promotional purposes of similar events in a tasteful manner. In this regard, I understand these images and/or recordings may be used in perpetuity, with or without edits to the originals.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

I AM ACKNOWLEDGING MY CONSENT TO ALL TERMS LISTED ABOVE. FURTHERMORE, I AM AGREEING THAT THE INFORMATION I HAVE SUBMITTED ON THIS FORM AND IN THE REGISTRATION PROCESS IS COMPLETE AND ACCURATE AND THAT I AM THE PARENT OR LEGAL GUARDIAN OF THE STUDENTS FOR WHOM I AM SIGNING.
